Information

Personal Injury and Worker’s Compensation

Heaven & Health accepts patients requiring treatment for injuries from workplace accidents (Worker’s Compensation) and from auto accidents (Personal Injury).

Worker’s Compensation

If you are injured on the job, you are usually covered by Worker’s Compensation under your employer.
You will need to fill out the necessary forms at work to obtain a claim number, then visit your doctor for an evaluation. He/she can provide you with a prescription for medical massage therapy.

Auto Accidents

Many people are unaware that injury symptoms following an automobile accident are often delayed. Common injuries include neck pain, whiplash, thoracic and back pain. If left untreated, these injuries can lead to a lifetime of problems and chronic pain. It is therefore, best to seek treatment as soon as possible after your accident. You will need to call your auto insurance company to obtain a claim number and ask your doctor for a prescription for medical massage therapy.

In both cases we will bill your insurance company directly. We will communicate with your physician, insurance company and (if necessary) your attorney. This allows you to focus on the most important thing, which is your recovery.

Call us today and let us help you get on that road to recovery!

Let us know if you have any questions. We are happy to help in any way we can!

Yes, we do accept insurance for medical massage related to auto accidents, worker’s compensation injuries and slip and fall cases. In most cases a doctor’s prescription is required for us to be able to bill your insurance company directly. Call us for more information.

Payment may be made with cash or cheque. All major credit cards are accepted. Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) may also be used.

Payment is expected at time of service, unless other arrangements have been made.

Details on how to save with our pre-payment plans can be found by clicking on the link below.

Gratuities are entirely at your discretion and are graciously accepted.

Gratuities may no longer be charged to a credit/debit card but may be given directly to the therapist, in the form of cash or by electronic cash transfer systems such as Venmo.

We use an unscented massage gel called Bon Vitale for most massages. This product is unscented, is easily absorbed and does not leave a greasy feel. Other oils/lotions are available on request or feel free to bring your own!

Massage should NEVER be painful and deep tissue work performed slowly with intention by a skilled therapist will be pain-free.
